Investment Report - August 2018 Commission District(s): Citywide 5. PUBLIC PARTICIPATION Any member of the public may speak to any issue that is not agendized for public hearing at this meeting. Speakers will be limited to three minutes during this item and at public hearings. There will be a thirty (30) minute aggregate time limit for this item, and speakers are encouraged to sign up in advance with the City Clerk prior to their participation. When an issue has been designated as quasi-judicial, public remarks shall only be heard during a quasi-judicial hearing that has been properly noticed for that matter. Approval of the October 10, 2018 Regular Commission Meeting Minutes Approval of the October 10, 2018 Regular Commission Meeting Minutes b, TR13186 - St. Malachy Annual Church Carnival: Fee Waiver -Extension of Hours Request- Alchohol Sales & Consumption Petition A Resolution of the City Commission of the City of Tamarac, Florida, authorizing the appropriate City Officials to grant a waiver of Business Revenue Licensing fees in the amount of $1,558.90, to extend the hours of operation to 11:00 p.m. on Thursday, November 1, 2018 through Saturday, November 3, 2018; and to permit the sale and consumption of alcoholic beverages on premise on Friday, November 1, 2018, through Sunday, November 4, 2018 for the Saint Malachy Catholic Church Annual Carnival, located at 6200 John Horan Terrace, beginning on Thursday, November 1, 2018 and ending on Sunday, November 4, 2018; providing for conditions of approval; providing for conflicts; providing for severability; and providing for an effective date. TR13193 - Accepting the Donation of Starting Platform for Aquatic Center A Resolution of the City Commission of the City of Tamarac, Florida, authorizing the appropriate City Officials to accept the donation of a single post Xcellerator Starting Platform from Spectrum Aquatics in an estimated amount of $5,500.00, for use at the Caporella Aquatic Complex pool; directing that the final actual cost will be recorded in the City's fixed asset inventory; providing for conflicts; providing for severability; and providing an effective date. d. TR13181 - Appointment to the Affordable Housing Advisory Committee A Resolution of the City Commission of the City of Tamarac, Florida, appointing members to the City's Affordable Housing Advisory Committee as required by Section 420.9076 (2) Florida Statutes and City Ordinance No. 2008-04; providing for the expiration of terms of office; directing City Staff to take any and all action necessary to effectuate the intent of this Resolution; providing for conflicts; providing for severability; and providing for an effective date. Commission District(s): Citywide e, TR13190 - Amendment 3 with Jeff Ellis Management A Resolution of the City Commission of the City of Tamarac, Florida, authorizing the appropriate City Officials to execute Agreement Amendment #3 between the City of Tamarac and Jeff Ellis Management, LLC, exercising the option to renew the Agreement to provide for Swimming Pool Management Services for the Caporella Aquatic Center, at a cost of $319,376.90 annually, plus shared revenues, for a two-year period effective January 1, 2019-December 31, 2020; approving funding from the appropriate accounts; providing for conflicts; providing for severability; and providing an effective date. f. TR13192 - Approving & Adopting Tamarac Transit 2018 Title VI Program Plan A Resolution of the City Commission of the City of Tamarac, Florida, authorizing the appropriate City Officials to approve and adopt the City of Tamarac Transit 2018 Title VI Program Plan; providing for conflicts; providing for severability; and providing for an effective date. g, TR13189 - Authorize an Agreement with Clean Harbors for Household Hazardous Waste Processing A Resolution of the City Commission of the City of Tamarac, Florida, approving collection and disposal services of household hazardous waste; authorizing an expenditure of funds for an amount not to exceed the approved Annual Budget; utilizing RFP No. 06-06-18-11 solicited and awarded by the City of Coconut Creek, Florida as the Lead Agency for the Southeast Florida Governmental Purchasing Cooperative Group; effective upon approval through October 23, 2020 or as extended by the City of Coconut Creek on behalf of the Southeast Florida Governmental Purchasing Cooperative; authorizing the proper City Officials to execute contract renewals; providing for conflicts; providing for severability; and providing for an effective date. h, TR13184 - HOME: Approval of Interlocal Agreement FY2017/2018 A Resolution of the City Commission of the City of Tamarac, Florida, accepting the 2017-2018 fiscal year Home Investment Partnership ("HOME") funds in the amount of $128,483 for a term retroactively commencing October 1, 2017 with an end date of September 30, 2020; authorizing the appropriate City officials to execute an Interlocal Agreement between Broward County and the City of Tamarac for the designation of subrecipient and providing for disbursement of HOME program funds for the Owner -Occupied Minor Home Repair Program, fiscal year 2017-2018, attached hereto as Exhibit "A", and incorporated herein; providing for conflicts; providing for severability; and providing for an effective date. Commission District(s): Citywide TR13191 - Select, Approve and Award Temporary Staffing Services for Filling Temporary Vacancies and Providing for Additional Staffing As Required A Resolution Of The City Commission Of The City Of Tamarac, Florida Authorizing The Appropriate City Officials To Select, Approve And Award The Acquisition Of Temporary Staffing Services On An As Needed Basis To Temporarily Fill Budgeted Employee Vacancies For All City Departments And Provide For A Method To Staff Temporary Positions As A Result Of Unanticipated Or Temporary Departmental Workload Increases In An Amount Not To Exceed An Aggregate Total Of $150,000.00 For Fiscal Year 2019, Utilizing Agreements Awarded By The State Of Florida Department Of Management Services, The South Florida Governmental Purchasing Cooperative Or Other Formally Competed Agreements By Other Florida Governmental Units, Or National Purchasing Cooperatives, And To .Establish A Process For Acquiring Specialized Temporary Staffing Needs, As Well As Establishing Quotation Procedures When There Are No Available Firms With Competitively Bid Agreements, In The Best Interest Of The City ; Providing For Conflicts; Providing For Severability; And Providing For An Effective Date. Commission District(s): Citywide k, TR13195 - Salt Purchase for Hypo Generation (Water Treatment Plant) A Resolution of the City Commission of the City of Tamarac, Florida, approving the purchase of solar salt from Morton Salt, Inc.; authorizing an expenditure of funds for an amount not to exceed the approved Annual Budget; utilizing ITB 2017-024 solicited and awarded by the City of Boca Raton, Florida as the Lead Agency for the Southeast Florida Governmental Purchasing Cooperative Group; effective upon approval through May 30, 2019 or as extended by the City of Boca Raton on behalf of the Southeast Florida Governmental Purchasing Cooperative; authorizing the proper City Officials to execute contract renewals; providing for conflicts; providing for severability; and providing for an effective date. TR13196 - Resolution Approving the Second Amendment to the RRB Settlement to Retain Public Ownership of the Alpha 250 Site A Resolution of the City Commission of the City of Tamarac, Florida, approving the Second Amendment to the Settlement Agreement with Broward County for the Litigation styled City of Sunrise et. al. vs. Broward County; authorizing the execution thereof; providing for severability; providing for conflicts; and providing for an effective date. Commission District(s): Citywide TR13200 - Approval of Agreement with Cynthia Fisher for Metamorphosis at Tamarac Village M. Public Art A Resolution of the City Commission of the City of Tamarac, Florida, approving the execution of an agreement between the City of Tamarac and Cynthia Fisher d/b/a Big Bang Mosaics attached hereto as Exhibit "T' and incorporated herein, to provide a mosaic work of art to be placed in the Tamarac Village Park located at 9090 and 9100 NW 57th Street, Tamarac, in the amount of $150,000; providing for conflicts; providing for severability; and providing an effective date. TR13197 - Health Insurance Renewal A Resolution of the City Commission of the City of Tamarac, Florida, authorizing the appropriate City officials to renew and execute an agreement with Cigna, providing for administrative services for claims and Flexible Spending Plans and the Employee Assistance Program, the Telehealth Connection Program and the Motivateme Value Program, and to make certain modifications to the Health Insurance program for employees and retirees; providing for conflicts; providing for severability; and providing for an effective date. 0. TR13198 - Stop Loss Insurance A Resolution of the City Commission of the City of Tamarac, Florida, authorizing the appropriate City officials to execute an agreement with Cigna, providing for Stop Loss Insurance; providing for conflicts, providing for severability; and providing for an effective date. P. TR13199 - Dental Insurance renewal A Resolution of the City Commission of the City of Tamarac, Florida, authorizing the appropriate City officials to renew and execute an agreement with Cigna, providing for a Dental Insurance Program, and to make certain modifications to the program for employees and retirees; providing for the continuation of the existing cost allocation of the dental insurance premium between the City and employees; providing for conflicts; providing for severability; and providing for an effective date. 7. REGULAR AGENDA 8. ORDINANCE(S) - FIRST READING a. T02386 - Amending Ordinance 2018-03 FY2018 Operating & Capital Budget An Ordinance of the City Commission of the City of Tamarac, Florida, amending Ordinance 2018- 03, which amended the City of Tamarac Operating Budget, Revenues and Expenditures, the Capital Budget, and the Financial Policies for the Fiscal Year 2018, by increasing the total revenues and expenditures by a total of $27,521,448, as detailed in Attachment attached hereto and summarized in Attachment B; providing for conflicts; providing for severability; providing for an effective date. 9. TB028: KFC - Special Exception Special Exception approval to allow for the use of a 1,984-square foot fast food restaurant with drive - through service (KFC) in a B-1 (Neighborhood Business District) now, MU-C (Mixed Use Corridor) Zoning District pursuant to the Tamarac Code of Ordinances. Commission District(s): District 2 b. TR13201 - KFC- Major Revised Site Plan A Resolution of the City Commission of the City of Tamarac, Florida, granting Major Revised Site Plan approval (with conditions) for KFC, to allow for the construction of a 1,984-square foot fast food restaurant with drive -through service for the subject property located at 7556 NW 58th Street (Case No. 6-SP-18); providing for conflicts; providing for severability; and providing for an effective date. Commission District(s): District 2 12. OTHER The City Commission may consider and act upon such other business as may come before it.
