RESOLUTION NO. R-2010- %
A RESOLUTION OF THE CITY COMMISSION OF
THE CITY OF TAMARAC, FLORIDA,
AUTHORIZING THE APPROPRIATE CITY
OFFICIALS TO RENEW THE COVERAGE FOR
THE CITY'S DENTAL INSURANCE PROGRAM
WITH UNITED HEALTHCARE FOR PLAN YEAR
EFFECTIVE JANUARY 1, 2011; PROVIDING FOR
PREMIUM RATE CHANGES; PROVIDING FOR
THE CONTINUATION OF THE EXISTING COST
ALLOCATION OF THE DENTAL INSURANCE
PREMIUM BETWEEN THE CITY AND
EMPLOYEES; PROVIDING FOR CONFLICTS;
PROVIDING FOR SEVERABILITY; AND
PROVIDING FOR AN EFFECTIVE DATE.
WHEREAS, the City's dental insurance was awarded to United Healthcare
and is scheduled to expire on December 31, 2010; and
WHEREAS, United Healthcare provided premium rates for the renewal of
the City's dental insurance plan for 2011; and
WHEREAS, the City has reviewed our experience with United Healthcare,
and determined that employees were satisfied with United Healthcare as its
insurance provider; and
WHEREAS, the City reviewed the City's plan design and determined that
the current plan design is appropriate and no plan design changes should be
made for 2011; and
WHEREAS, the City staff has worked with representatives of Willis of
Florida and United Healthcare to negotiate the most comprehensive and cost
effective dental plan for the City's employees and their dependents; and

WHEREAS, after completion of negotiations, the overall dental insurance
premiums will represent an 8% increase, with no change in premium for the
Dental HMO (DHMO) plan and an increase of 30% for the Dental PPO (DPPO)
plan, as described in Exhibit #1 attached hereto; and
WHEREAS, the City proposes to continue the existing cost allocation of
dental premiums between the City and employees; and
WHEREAS, available funds exist in the appropriate Governmental Funds
which are in the approved FY2011 Budget; and
WHEREAS, the City Commission has deemed it to be in the best interest
of the health, safety and welfare of citizens and residents of the City of Tamarac
to execute the dental insurance contract with United Healthcare for dental
insurance for City of Tamarac employees, as outlined in Exhibit #1, attached
hereto, subject to any revisions consistent with the benefit plan as may be
negotiated by and between City staff and United Healthcare and as approved by
the City Manager and the City Attorney effective January 1, 2011.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE
CITY OF TAMARAC, FLORIDA:
SECTION 1: That the foregoing "WHEREAS" clauses are hereby
ratified and confirmed as being true and correct and are hereby made a specific
part of this Resolution. All exhibits attached hereto are incorporated herein and
made a specific part of this Resolution.

SECTION 2: That the appropriate City officials are hereby
authorized to approve the renewal of the City's dental insurance program with
United Healthcare for City of Tamarac employees as outlined in Exhibit #1,
attached hereto, subject to any revisions consistent with the benefit plan as
may be negotiated by and between City staff and United Healthcare and as
approved by the City Manager and the City Attorney effective January 1, 2011.
SECTION 3: That the appropriate City officials are hereby
authorized to continue the existing cost allocation of the dental insufance
premium between the City and the employees.
SECTION 4: That all resolutions or parts of resolutions in conflict
herewith are hereby repealed to the extent of such conflict.
SECTION 5: That if any clause, section, or other part or
application of this Resolution is held by any court of competent jurisdiction to be
unconstitutional or invalid, in part or application, it shall not affect the validity of
the remaining portions or applications of this Resolution.

SECTION 6: This Resolution shall become effective immediately
upon adoption.
