CITY OF TAMARAC, FLORIDA
RESOLUTION NO R2008-
A RESOLUTION OF THE CITY COMMISSION OF THE
CITY OF TAMARAC, FLORIDA, AUTHORIZING THE
APPROPRIATE CITY OFFICIALS TO PURCHASE
EMERGENCY MEDICAL SERVICES (EMS) COMPUTER
SOFTWARE ($37,750.00), TRAINING ($4,800.00) AND
ANNUAL WARRANTY ($4,795.00) FROM DOCUMED
SYSTEMS INTERNATIONAL, INC. IN THE AMOUNT OF
FORTY SEVEN THOUSAND THREE HUNDRED FORTY
FIVE ($47,345.00) DOLLARS; PROVIDING FOR
CONFLICTS; PROVIDING FOR SEVERABILITY; AND
PROVIDING FOR AN EFFECTIVE DATE.
WHEREAS, the City of Tamarac through the Fire Department provides
Emergency Medical Services (EMS) and Advanced Life Support (ALS) transport;

WHEREAS, the City of Tamarac accepted an EMSTARS Assistance
Grant (Resolution #: R-2008-75) in the amount of $62,750.00 for the purchase of
computer hardware ($27,750.00) and software ($35,000.00) for an electronic
patient data management system; and
WHEREAS, DocuMed Systems International, Inc. was used in the State of
Florida alpha test of its EMSTARS program which gave the company advanced
testing of EMSTARS and compliance with the Federal data program; and
WHEREAS, DocuMed Systems International, Inc. has had an interface
with Tamarac Fire Rescue's billing agent, ADPI billing systems, for over seven

years and program software is utilized by numerous EMS agencies within the
State; and
WHEREAS, funds for these purchases have been budgeted in the Fire -
Rescue FY08 budget, Project #GT08E, and through the Florida Bureau of
Emergency Medical Services (EMS) grant award, and
WHEREAS, DocuMed has agreed to honor the pricing structure listed in
their original quote dated March 28, 2006, attached hereto as Exhibit "A",- and
WHEREAS, the Fire Chief and Purchasing Contract Manager
recommends the computer software purchase from DocuMed Systems
International, Inc., which is to be in the best interest of the City; and
WHEREAS, the City Commission of the City of Tamarac, Florida deems it
to be in the best interest of the citizens and residents of the City of Tamarac to
approve the computer software purchase from DocuMed Systems International.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF
THE CITY OF TAMARAC, FLORIDA:
SECTION 1: The foregoing "WHEREAS" clauses are hereby
ratified and confirmed as being true and correct and are hereby made a specific
part of this Resolution.
SECTION 2: The Commission authorizes the appropriate City
officials a purchase in the amount of $47,345.00 from DocuMed Systems
International, Inc.
SECTION 3: All resolutions or parts of resolutions in conflict
herewith are hereby repealed to the extent of such conflict.

SECTION 4: If any clause, section, other part or application of this
Resolution is held by any court of competent jurisdiction to be unconstitutional or
invalid, in part or application, it shall not affect the validity of the remaining
portions or applications of this Resolution.
SECTION 5: This Resolution shall become effective immediately upon its
passage and adoption.
