RESOLUTION NO. R-2004- ,;,3l
A RESOLUTION OF THE CITY COMMISSION OF
THE CITY OF TAMARAC, FLORIDA, AUTHORIZING
THE APPROPRIATE CITY OFFICIALS TO
APPROVE THE RENEWAL OF LONG-TERM
DISABILITY INSURANCE COVERAGE WITH UNUM
AT THE PRESENT RATE THE CITY PAYS FOR
INSURANCE COVERAGE FOR EMPLOYEES IN
THE APPOINTED, EXECUTIVE,
MANAGERIAL/PROFESSIONAL AND
ADMINISTRATIVE CATEGORIES WHO
PARTICIPATE IN THE CITY'S 401(a) RETIREMENT
PLAN EFFECTIVE JANUARY 1, 2005; PROVIDING
FOR THE CONTINUATION OF THE EXISTING
COST ALLOCATION OF THE LONG-TERM
DISABILITY INSURANCE PREMIUM BETWEEN
THE CITY AND EMPLOYEES; PROVIDING FOR
CONFLICTS; PROVIDING FOR SEVERABILITY;
AND PROVIDING FOR AN EFFECTIVE DATE.
WHEREAS, the long-term disability insurance was awarded to UNUM effective
January 1, 1998, renewing on January 1, 2005; and
WHEREAS, the rates for long-term disability coverage will remain the same, at
$.45 per $100 of covered payroll this renewal period, January 1, 2005 through
December 31, 2005 as indicated in the letter from UNUM dated September 29, 2004
attached hereto as Exhibit #1; and
WHEREAS, the Human Resources Director recommends the renewal of the
long-term disability insurance coverage with no increase in premium rates effective
January 1, 2005 for employees in the Appointed, Executive, Managerial/Professional
and Administrative categories who participate in the City's 401(a) retirement plan; and

WHEREAS, available funds exist in the appropriate funds which are in
the approved FY2005 Budget; and
WHEREAS, the City Commission has deemed it to be in the best interest of the
citizens and residents of the City of Tamarac to provide long-term disability insurance
benefits for the City employees.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE CITY
OF TAMARAC, FLORIDA:
SECTION 1: That the foregoing "WHEREAS" clauses are hereby
ratified and confirmed as being true and correct and are hereby made a specific part of
this Resolution.
SECTION 2: That the appropriate City officials are hereby authorized to
approve the renewal of UNUM as the City's carrier for long-term disability insurance
coverage for City of Tamarac employees in the Appointed, Executive,
Managerial/Professional and Administrative categories who participate in the City's
401(a) retirement plan with no increase in premium rates effective January 1, 2005.
SECTION 3: That the appropriate City officials are hereby authorized to
continue the existing cost allocation of the long-term disability insurance premium
between the City and the employees in the Appointed, Executive,
Managerial/Professional and Administrative categories who participate in the City's
401(a) retirement plan.
SECTION 4: That all resolutions or parts of resolutions in conflict
herewith are hereby repealed to the extent of such conflict.
SECTION 5: That if any clause, section, or other part or application of
this Resolution is held by any court of competent jurisdiction to be unconstitutional or

invalid, in part or application, it shall not affect the validity of the remaining portions or
applications of this Resolution.
SECTION 6: This Resolution shall become effective immediately upon
adoption.
PASSED, ADOPTED AND APPROVED THIS 27th day of October, 2004.
