CITY OF TAMARAC, FLORIDA
RESOLUTION NO. R-99- 3 3
A RESOLUTION OF THE CITY COMMISSION OF THE
CITY OF TAMARAC, FLORIDA, AUTHORIZING THE
APPROPRIATE CITY OFFICIALS TO EXECUTE AN
AGREEMENT BETWEEN THE CITY OF TAMARAC AND
MEDTRONICS PHYSIO-CONTROL CORPORATION, A
SOLE SOURCE, FOR THE PURCHASE OF ONE (1) LIFE
PAK 12 CARDIAC DEFIBRILLATORS AT A COST OF
$16,917.98; FOUR (4) FAST PAK BATTERIES WITH FUEL
GAUGE AT A COST OF $714.40; ONE (1) POINT OF SALE
SERVICE AGREEMENT AT A COST OF $864.00, FOR A
TOTAL COST OF $18,496.38; PROVIDING FOR
CONFLICTS; PROVIDING FOR SEVERABILITY; AND
PROVIDING FOR AN EFFECTIVE DATE.
WHEREAS, the State of Florida requires Cardiac Defibrillators on every Advanced
Life Support Vehicle; and
WHEREAS, the City of Tamarac Fire Rescue Department wishes to purchase one
(1) life pak 12 cardiac defibrillators at a cost of $16,917.98; four (4) fast pak batteries with
fuel gauge at a cost of $714.40; one (1) point of sale service agreement at a cost of
$864.00; and
WHEREAS, Medtronics Physio-Control Corporation will provide one (1) basic
carrying case, one (1) left pouch, one (1) back pouch and one (1) front cover at no charge;
and

WHEREAS, Physio-Control Corporation is a sole source; and WHEREAS,
funds for this purchase have been budgeted in the FY00 Fire Rescue Department;
WHEREAS, it is the recommendation of the Fire Chief and the Purchasing and
Contracts Manager that this purchase be awarded to Medtronics Physio-Control
Corporation; and
WHEREAS, the City Commission of the City of Tamarac, Florida deems it to be in
the best interests of the citizens and residents of the City of Tamarac that the City
purchase one (1) Cardiac Defibrillator and monitor from Medtronics Physio-Control
Corporation.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE
CITY OF TAMARAC, FLORIDA:
SECTION 1: The foregoing "WHEREAS" clauses are hereby ratified and confirmed
as being true and correct and are hereby made a specific part of this Resolution.
SECTION 2: The appropriate City officials are hereby authorized to execute
attached Agreement (Exhibit "B") and to purchase one (1) Life Pak 12 Cardiac Defibrillator,
four (4) Fast Pak Batteries with fuel gauge and one (1) Point of Sale Service Agreement
for a total cost of $18,496.83 from Medtronics Physio-Control Corporation.
SECTION 3: All resolutions or parts of resolutions in conflict herewith are hereby
repealed to the extent of such conflict.

SECTION 4: If any clause, section, other part or application of this Resolution is
held by any court of competent jurisdiction to be unconstitutional or invalid, in part or
application, it shall not affect the validity of the remaining portions or applications of this
Resolution.
SECTION 5: This Resolution shall become effective immediately upon its passage
and adoption.
