R-96-� A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF TAMARAC, FLORIDA, AUTHORIZING THE APPROPRIATE CITY OFFICIALS TO EXECUTE AN AGREEMENT CONCERNING THE SELECTION OF A HEALTH INSURANCE PROVIDER FOR CITY EMPLOYEES EFFECTIVE FEBRUARY 1, 1996; PROVIDING FOR CONFLICTS; PROVIDING FOR SEVERABILITY; AND PROVIDING FOR AN EFFECTIVE DATE. 1 WHEREAS, the Agreement between the City of Tamarac and Humana Health Care Plans expires January 31, 1996; and WHEREAS, a Request for Proposal 95-09 entitled "Health Benefits Program" was issued and in response six (6) proposals were received, however, one proposal was rejected for failure to sign the proposal; and WHEREAS, the City of Tamarac created a Health Review Committee to perform an analysis of the proposals submitted and recommend a Health Insurance Provider; and WHEREAS, the Health Committee consisted of the Risk Manager, Benefits Coordinator, Budget Officer, Personnel Analyst, Purchasing Officer, Senior Planner Community Development, Operations Manager Public Works, IAFF Representative and FPE Representative; and z Temp. Reso. 7269 January 4, 1996 WHEREAS, each company was invited to present an overview of their proposal to the committee; and WHEREAS, after careful evaluation, the committee narrowed the selection to CAC United Health Plans and HIP Health Plans; and WHEREAS, it is the recommendation of the Health Review Committee to approve CAC -United HealthCare Plans of Florida as the Health Insurance Provider; and WHEREAS, after an independent evaluation, the City Manager concurs with this recommendation that the City of Tamarac enter into an agreement with CAC United HealthCare Plans of Florida to provide Health Insurance coverage for City of Tamarac employees; and WHEREAS, the City Manager recommends adopting a percentage of contribution, 100% for HMO/POS single and 80% of all other coverages, which standardizes the treatment of City employees; and WHEREAS, the City Commission of the City of Tamarac, Florida deems it to be in the best interests of the citizens and residents of the City of Tamarac to approve "IP i+eA t4\2I,ak,S as the Health Insurance Provider and provide for its proper execution. 3 Temp. Reso.p9 January 4, 1996 NOW, THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE CITY OF TAMARAC, FLORIDA: SECTION 1: That the foregoing "WHEREAS" clauses are hereby ratified and confirmed as being true and correct and hereby made a specific part of this Resolution. SECTION 2: That the appropriate City officials are hereby authorized to accept and execute an agreement between the City of Tamarac, Florida, and 141 P RU Ilh &M S _ _ _ to provide Health Insurance for the employees of the City of Tamarac effective February 1, 1996. SECTION 3: That all appropriate budget transfers required to fund the provisions of this Resolution are hereby approved. BECTION 4: That the City Commission agrees with the City Manager's recommendation on cost allocation and hereby appropriates the funds necessary from an account to be designated by the City Manager. SECTION 5: All Resolutions or parts in conflict herewith are hereby repealed to the extent of such conflict. . SECTION 6: If any clause, section, or other part of application of this Resolution is held by any court of competent jurisdiction to be unconstitutional or invalid, in part or application, it shall not affect the validity of the remaining portions or application of this 4 Temp.
