R-96 " ,, �JeZ A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF TAMARAC, FLORIDA, AUTHORIZING THE APPROPRIATE CITY OFFICIALS TO EXECUTE ADDENDUM NUMBERS 95-11, 95-12, 95-13 AND 95-14 TO THE CONTRACT WITH HAZEN & SAWYER, P.C., TO PROVIDE DESIGN, BIDDING, PERMITTING AND CONSTRUCTION DOCUMENTS FOR THE INSTALLATION OF WATER AND WASTEWATER UTILITIES ASSOCIATED WITH FOUR FY96 CAPITAL IMPROVEMENT PROJECTS FOR A PROPOSED AMOUNT NOT TO EXCEED $136,850; PROVIDING FOR FUNDING FROM THE APPROPRIATE BUDGETED UTILITIES OPERATIONAL AND CIAC ACCOUNTS; PROVIDING FOR CONFLICTS; PROVIDING FOR SEVERABILITY; AND PROVIDING FOR AN EFFECTIVE DATE. WHEREAS, the City Commission approved Capital Improvement Projects in the FY96 Budget on September 27, 1995; and WHEREAS, the FY 96 Budget contains Utilities Capital Improvement Projects (C.I.P.'s) that require the services of the City Consulting Engineers; and WHEREAS, the Utilities Department has requested Hazen & Sawyer, P.C., one of the City's Consulting Engineers, to present proposals for design, bidding,permitting and construction documents for four of the FY96 C.I.P.'s in accordance with the City Consulting Engineering Agreement as authorized by the City on February 23, 1994, Resolution No. 1 1 Temp. Reso. #7324 Revised 2/14/96 Revised 2/15/96 R-94-32; and WHEREAS, Hazen & Sawyer, P.C. has submitted the following Addenda to the Agreement: 1) Addendum No. 95-11, Pump Stations 12 & 22 Rehabilitation (Survey, Design, Permit, Bidding and Construction Services) (Attached hereto as $ 40,850 "Exhibit 1" and in map form as "Exhibit 2" and "Exhibit 3") 2) Addendum No. 95-12, Shaker Village Water Main Replacement (Survey, Design, Permit and Bidding Services) (Attached hereto as "Exhibit 4" and in $ 68,000 map form as "Exhibit 5") 3) Addendum No. 95-13, Pump Station 11A Force Main Relocation (Design, Permit, Bidding and Construction Services) (Attached hereto as $ 18,500 "Exhibit 6" and in map form as "Exhibit 7") 4) Addendum No. 95-14, Galvanized Services Replacement (Design and Bidding Services) $ 9,500 (Attached hereto as "Exhibit 8" and in map form as "Exhibit 9") TOTAL COMPENSATION $ 136,850; and WHEREAS, the costs of the engineering services for the construction documents of the water and wastewater utilities will be funded from the appropriate budgeted Utilities Operational and Contribution in Aid of Construction (CIAC) Accounts; and 2 Temp. Reso. #7324 Revised 2/14/96 Revised 2/15/96 WHEREAS, it is the recommendation of the Director of Utilities that Addendum Numbers 95-11, 95-12, 95-13 and 95-14 providing for the design, bidding, permitting and construction documents for the installation of City Utilities for these four FY96 C.I.P.'s be approved and executed; and WHEREAS, the City Commission of the City of Tamarac, Florida deems it to be in the best interest of the citizens and residents of the City of Tamarac to accept and execute Addendum Numbers 95-11, 95-12, 95-13 and 95-14 to the Engineering Agreement with Hazen & Sawyer, P.C. to provide design, bidding, permitting and construction documents for the installation of the water mains, force mains and lift station rehabilitation associated with: 1) Pump Stations 12 and 22 Rehabilitation; 2) Shaker Village Water Main Relocation; 3) Pump Station 11A Force Main Relocation; and 4) Galvanized Services Replacement for a proposed amount not to exceed $136,850. NOW, THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE CITY OF TAMARAC, FLORIDA: SECTION 1: The foregoing "WHEREAS" clauses are hereby ratified and confirmed as being true and correct and are hereby made a specific part of this resolution. 3 Temp. Reso. #7324 Revised 2/14/96 Revised 2/15/96 SECTION 2: That the appropriate City Officials are hereby authorized to execute Addendum Numbers 95-11, 95-12, 95-13 and 95-14 with Hazen & Sawyer, P.C., to provide design, bidding, permitting and construction documents for the installation of water and wastewater utilities associated with four Capital Improvement Projects for a proposed amount not to exceed $136,850 in accordance with the City Consulting Engineering Agreement as authorized by Resolution No. R-94-32 (attached hereto as "Exhibit 10") SECTION 3: That $136,850 is approved to be funded from the appropriate budgeted Utilities Operational and CIAC Accounts. SECTION_ All resolutions or parts of resolutions in conflict herewith are hereby repealed to the extent of such conflict. SECTION 5: If any clause, section, other part or application of this Resolution is held by any court of competent jurisdiction to be unconstitutional or invalid, in part or application, it shall not affect the validity of the remaining portions or applications of this Resolution. 1 1 0 Temp. Colombia le, HuEN AND SAWYER ADDENDUM NO, 95-11 December 21, 1995 CITY OF TAMARAC PUMP STATIONS 12 AND 22 REHABILITATION TASK SCHEDULE Task Time of Order General Description Performance(') Compensation(2) 1 SITE SURVEY 14 Days $850 Perform site surreys of Pump Stations 12 and 22. Surveys shall identify property boundary and all physical improvements on each site. The City will provide plats for each site as available. 2 DESIGN 60 Days $25,000 Contract Documents shall be prepared for the rehabilitation of Pump Stations 12 and 22 to be bid as a single project. The rehabilitation includes the conversion of existing wet (concrete)/dry pit (steel) type station to a submersible type station. The existing dry pit will be abandoned. The City will provide specifications and drawings on disk from the Lift Stations 16 and 17 Rehabilitation project. The City will provide all available background drawings of Pump Stations 12 and 22. Hazen and Sawyer shall prepare a review set of contract documents to the City at the 70 percent stage. The final review submittal shall also be utilized as the permitting submittal and the City Plan / Specification Review Committee submittal. 3 BCDNRP PERMIT APPLICATION 105 Days $3,000 Hazen and Sawyer, P.C., shall prepare and submit permit application to Broward County Department of Natural Resources Protection (BCDNRP) for the rehabilitation of the lift stations, attend one meeting with City / BCDNRP to discuss application and respond to BCDNRP questions. Included is the cost for permit application fees (BCDNRP-$745.00 and FDEP-$250.00). Operational permits and associated fees and as-builts are not part of this Task. Page 2of4 Hwd.8619C002 RVEN AND SAWYER ADDENDUM NO. 95-11 December 21, 1995 Task Time of Order General Description Performance(') Compensation(2) 4 BIDDING AND AWARD 120 Days $3,000 Hazen and Sawyer shall prepare and print bidding documents, entertain questions from bidders, conduct a prebid conference, prepare addenda as required, and prepare a bid tabulation. The City will sell and distribute bidding documents to prospective bidders and suppliers. The City will review the bid proposals and check bidder responsiveness. Hazen and Sawyer shall check the references of the lowest responsive bidder and prepare a recommendation for award. Hazen and Sawyer shall transmit to the City the plans and specifications on computer disk and one set of printing originals of the plans and specifications to the City. 5 CONSTRUCTION SERVICES 245 Days $9,000 Task 5 is for construction services after the award of a contract to a general contractor for the rehabilitation of Pump Stations 12 and 22. The intent of this Task is to certify completion for the regulatory agencies. Hazen and Sawyer will perform a limited number of site visits (up to six visits per station) to verify construction for project certifications. Hazen and Sawyer shall attend the preconstruction conference and answer contractor questions directed through the City. Daily construction services activities, pay request verification, etc. will be performed by the City. As -built drawings shall be prepared using information (dimensions, elevations and changes) provided by the City and the Contractor. Hazen and Sawyer shall certify the project completion and complete appropriate application forms on the City's behalf required by the regulatory agencies. Fees associates with filing of such forms shall be paid for directly by the City. Total 245 Days $40,850 Notes: Time of performance is measured subsequent to the notice to proceed, which shall be considered as the date of award of a construction contract to a contractor. Colombia 9�� HAZEN AND SAWYER ADDENDUM NO. 95-12 January 15, 1996 • CITY OF TAMARAC SHAKER VILLAGE WATER MAIN REPLACEMENT TASK SCHEDULE Task Time of Order General Description Performance(') Compensation(2) PREAMBLE The Shaker Village Area (between Commercial Boulevard, Bailey Road and east of The Commons road to the canal) has been identified by the City to contain water mains and service connections that are excessively deep and are difficult to maintain. This project will provide the means to raise the existing water facilities to current Utilities Department Standards. SURVEYING 30 Days $14,000 The site shall be surveyed to include all areas to receive new water mains and service connections. This includes all roadways, driveways and areas up to the existing sidewalks. Existing topographic features and utility information (valve boxes, meter boxes, manholes and inverts, etc.) shall be identified. It is believed that there are no existing utility easements. The location of and dedication of easements will be by the City. 2 DESIGN - PHASE 1 AND 2 80 Days $42,000 Contract Documents shall be prepared for the rehabilitation of the installation of a new water main and service connections in the Shaker Village Area. The project shall be designed to be bid as two separate construction bid packages. Hazen and Sawyer shall provide an initial Budget Cost Estimate of the project. The limits of the bid packages shall be based on the estimate and available funds for construction. The City will provide background drawings of the existing site and the standard City stand front end specifications. Hazen and Sawyer shall prepare a review set of contract drawings identifying the proposed location of the water mains, and a review set of the contract documents to the City at the 70 percent stage. The final review submittal shall also be utilized as the permitting submittal and the City Plan / Specification Review Committee submittal. The relocation consists of tapping the existing 18-inch force main at Commercial Boulevard, isolation valve for the unused remaining force main, the selection and design of new pumps for LS 11 A and the selection of a design point for the private lift station on the same force main. A site survey of the area around the proposed tie in location of LS 11 A force main at Commercial Boulevard shall be performed identifying Right-of-way lines, easements and topographic features. No survey work is included at Lift Station 11A. City standard electrical panel drawings shall be used if modifications require panel replacement. The City will provide a set of specifications on disk. The City will provide all available background drawings of Pump Stations 11 A. Hazen and Sawyer shall prepare a review set of contract documents to the City at the 70 percent stage. The final review submittal shall also be utilized as the permitting submittal and the City Plan / Specification Review Committee submittal. 2 PERMITTING 75 Days $2,500 Hazen and Sawyer, P.C., shall prepare and submit permit applications to: 1) the Florida Department of Transportation (FDOT) for work in the Commercial Boulevard Right -of -Way and 2) Broward County Department of Natural Resources Protection (BCDNRP) for the relocation of the force main and installation of new pumps at LS 11A, attend one meeting with City / BCDNRP to discuss application and respond to FDOT and BCDNRP questions. Included is the cost for permit application fees (estimated fees are BCDNRP - $600.00 46 and FDEP - $250.00). Operational permits and associated fees and as-builts are not part of this Task. Hazen and Sawyer shall check the references of the lowest responsive bidder and prepare a recommendation for award. Hazen and Sawyer shall transmit to the City the plans and specifications on computer disk and one set of printing originals of the plans and specifications to the City. 4 CONSTRUCTION SERVICES 245 Days $5,000 Task 4 is for construction services after the award of a contract to a general contractor for the relocation of LS 11A force main. The intent of this Task is to certify completion for the regulatory agencies. Hazen and Sawyer will perform a limited number (up to three) of site visits to verify construction for project certifications. Hazen and Sawyer shall attend the preconstruction conference and answer contractor questions directed through the City. Daily construction services activities, pay request verification, etc. will be performed by the City. As -built drawings shall be prepared using information (dimensions, elevations and changes) provided by the City and the Contractor. Hazen and Sawyer shall certify the project completion and complete appropriate application forms on the City's behalf required by the regulatory agencies. Fees associates with filing of such forms shall be paid for directly by the City.
