A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF
TAMARAC, FLORIDA, AUTHORIZING THE APPROPRIATE CITY
OFFICIALS TO APPROVE ADDENDUM NO.94-05 OF THE HAZEN
& SAWYER ENGINEERING AGREEMENT IN THE AMOUNT OF
$17,388.00 REQUIRED FOR THE MUNICIPAL PARKING LOT
CONTAMINATION ASSESSMENT REPORTS; AUTHORIZING
FINANCE TO TRANSFER $17,388.00 FROM GENERAL
CONTINGENCY ACCOUNT NO. 001-872-588-975 TO SPECIAL
ACCOUNT NO. 155-654-539-4YE; PROVIDING FOR CONFLICTS;
PROVIDING FOR SEVERABILITY; AND PROVIDING FOR AN
EFFECTIVE DATE.
WHEREAS, the Florida Department of Environmental Protection required the City
of Tamarac to prepare a Contamination Assessment and a Contamination Assessment
Report on the Tamarac Municipal Parking Lot site by July 1, 1994; and
WHEREAS, the Tamarac City Council awarded the task of performing the
Contamination Assessment and the Contamination Assessment Report to Hazen &
Sawyer under Addendum No. 94-02 to the Engineering Agreement by Council Action on
March 23, 1994; and
WHEREAS, in the course of performing the Contamination Assessment, additional
work outside of the approved tasks listed in the Hazen & Sawyer Addendum No. 94-02
was required to submit the Contamination Assessment Report to Florida Department of
Environmental Protection; and

WHEREAS, upon receipt of a letter from Broward County Department of Natural
Resource Protection requesting additional information, the need for a Contamination
Assessment Report Addendum was required; and
WHEREAS, Hazen & Sawyer, Consulting Engineer for the City of Tamarac, has
submitted an Addendum No. 94-05 to the Engineering Agreement in the amount of
$17,388.00 which addresses the additional work required for the Contamination
Assessment Reports; and
WHEREAS, Finance will be required to transfer $17,388.00 from General
Contingency Account No. 001-872-588-975 to Special Account No. 155-654-539-4YE; and
WHEREAS, the Director of Public Services recommends approval.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY
OF TAMARAC, FLORIDA:
SECTION 1: That the appropriate City Officials are hereby authorized to approve
Addendum No. 94-05 of the Hazen & Sawyer Engineering Agreement in the amount of
$17,388.00 for the additional work required for the Tamarac Municipal Parking Lot
Contamination Assessment Reports, a copy of the Addendum (Exhibit"1 ") and Agreement
(Exhibit "2") are attached hereto and made a part hereof.
SECTION 2: That Finance be authorized to transfer $17,388.00 from General
Contingency Account No. 001-872-588-975 to Special Account No. 155-654-539-4YE.

SECTION 3: All resolutions or parts of resolutions in conflict herewith are hereby
repealed to the extent of such conflict.
SECTION 4: If any clause, section, other part or application of this resolution is
held by any court of competent jurisdiction to be unconstitutional or invalid, in part or
application, it shall not affect the validity of the remaining portions or applications of this
resolution.
SECTION 5: This resolution shall become effective immediately upon its passage
and adoption.

In accordance with your instruction, we are pleased to submit this Task Order Authorization
(Addendum No. 94-05) to provide engineering services as described hereinafter. It is understood
that said services will be provided in accordance with our City Consulting Engineering Services
Agreement with the City, as authorized by the City on February 23, 1994. Except as may be
explicitly revised or amended below for application specifically to this Addendum, all provisions
of said Agreement shall apply. In accordance with Section I of the Agreement, ENGINEER has
assigned Job Number 8603 to the task(s) included under this Addendum.

MUNICIPAL PARKING LOT - PHASE 2, PART B
TASK SCHEDULE
Task
Order General Description Com ensatlon(')
This Scope of Work addresses services related to the
Contamination Assessment (CA) submitted to the Florida
Department of Environmental Protection (FDEP) for the former
Municipal Parking lot. The scope has been developed to address
comments issued by the Broward County Department of Natural
Resources Protection (BCDNRP) in a letter to the City dated
August 10, 1994. Hazen and Sawyer will retain the services of
Geraghty and Miller, Inc. (G&M) to submit a Contamination
Assessment Report Addendum (CARA) to the BCDNRP. This work
is being conducted to support the City's acceptance in Florida's
is Abandoned Tank Restoration Program which, in turn, will provide
the CITY for reimbursement for allowable costs contained herein.

Because of the relatively high concentrations of volatile organic
constituents detected in water from Monitor Wells MW-4, MW-5,
and MW-6 (not including MW-7), elevated detection limits resulted
from organic interferences during the analysis by EPA Test Method
601. The subcontracted analytical laboratory utilized during the
Phase 2 CA has been notified of the BCDNRP concerns and is
preparing a formal response to this item, explaining the cause of
the elevated detection limits. In essence, the phenomena is not
correctable and resampling the monitor wells most likely will not
serve to "improve" the detection limits. For this reason, we request
the City contest the issue with the BCDNRP and not (at this time)
conduct the resampling they have asked for. The laboratory
response shall be attached to the CARA. If BCDNRP does not
accept this position, Task 6 would require authorization.
ADDITIONAL PHASE 2 MONITORING $1,400.00
For performing additional monitoring required to complete the
Contamination Assessment Report (CAR) for the groundwater
hydrocarbon contamination at the City's Municipal Parking Lot.
Included is the drilling of five additional monitor wells, and
associated sampling and analysis by G&M in order to identify the
extent of the contamination plume.

2 MONITOR WELL INSTALLATION AND SAMPLING $1,712.00
G&M shall install one hand augered soil boring in the previously
excavated and backfilled area of the former underground storage
tank (UST). Soil samples shall be obtained down to the water table
and shall be tested for organic vapor concentrations. The services
of a well drilling contractor shall be enlisted to install one shallow
(14-foot deep) monitor well in the position of the soil boring. During
this mobilization, a water sample shall be collected from Well DMW-
1, as requested in Item 4 of the BCDNRP letter. The water sample
shall be analyzed by a subcontracted laboratory utilizing EPA
Method 602.
• 3 WATER LEVEL MEASUREMENT $535.00
During the mobilization for Task 2, the G&M shall measure the
water level in all of the monitor wells at the site, including the
newly -installed shallow monitor well. All water -level measurements
shall be converted to groundwater elevations and the data shall be
tabulated. The top -of -casing of the new monitor well shall be
surveyed by a professional surveyor licensed in the State of
Florida.
4 ADDITIONAL DEEP MONITOR WELL $4,601.00
If the resampling of DMW-1 conducted during Task 2 indicates that
concentrations of volatile organic constituents are present above
allowable regulatory concentrations, then an additional deep
monitor well (DMW-2) shall be installed. This well shall be installed
to a depth of 55 feet with a screen from 50 to 55 feet. The well
shall be installed utilizing a temporary surface casing to prevent
potential migration of organic constituents from shallower depths to
the new monitoring zone. Within one week after DMW-2 is
installed, a water sample collected from the well will be analyzed by
EPA Methods 601, 602, and 239.2, which were those methods that
indicated detectable concentrations in water from DMW-1 during
the CA. Included in this task are the drilling and monitoring
associated with two deep wells.

General Description
REPORT ADDENDUM PREPARATION
Upon completion of Tasks 1 through 4 (as necessary) in this Scope
of Work, the CARA shall be prepared summarizing the
methodologies, results, and conclusions from the work tasks..
Figures and tables shall be prepared and contained within the
CARA to display the information. The CARA shall be signed and
sealed by a Professional Geologist and submitted to the BCDNRP.

6 MONITORING CONTINGENCY $7,000.00
This contingency item shall only be authorized if additional testing
or analysis is required in response to the CARA. Included in this
is task is the drilling and monitoring of up to three additional wells. A
separate Notice to Proceed will be required to be issued by the City
prior to commencement of work associated with this Task.
