CITY OF TAMARAC, FLORIDA
RESOLUTION NO. R-94-d "//
A RESOLUTION OF THE CITY COMMISSION
OF THE CITY OF TAMARAC, FLORIDA,
AUTHORIZING A 4% RATE INCREASE TO
THE CRAWFORD & COMPANY FEE
SCHEDULE FOR ADMINISTERING THE CITY'S
SELF -INSURED WORKERS' COMPENSATION
CLAIMS; PROVIDING FOR CONFLICTS;
PROVIDING FOR SEVERABILITY; AND
PROVIDING FOR AN EFFECTIVE DATE.
WHEREAS, the City of Tamarac, Florida deems it to be in the best interests of the
citizens and residents of the City to have its self -insured workers' compensation claims
administered by Crawford & Company, a professional third party administrator.
WHEREAS, Crawford & Company has requested a 6% rate increase and the City's
Risk Manager has negotiated a 4% increase to Crawford's fee schedule effective
December 1, 1994.
WHEREAS, Crawford & Company's projected fees for the year beginning December
1, 1994 are $12,000 - $13,000.
WHEREAS, Crawford & Company's annual fee will be $546.00 for each claim that
results in the payment of medical and disability benefits and the annual fee will be $494.00
for any claims that remain open after one year plus a consumer price index adjustment
after the third year.
WHEREAS, Crawford & Company's annual fee will be $76.50 for each claim that
results in the payment of medical benefits only plus a consumer price index adjustment for
claims that remain open after the third year.

WHEREAS, Crawford & Company's annual fee will be $494.00 for each claim that
subrogation is pursued against another party plus a consumer price index adjustment for
subrogation claims that are pursued after the third year.
WHEREAS, Crawford & Company will charge time (prevailing hourly fees) and
expenses for each claim that remains open after five years.
WHEREAS, the Risk Manager recommends approval of the new fees effective
December 1, 1994.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE CITY OF
TAMARAC, FLORIDA:
SECTION 1: The foregoing "WHEREAS" clauses are hereby ratified and confirmed as
being true and correct and are hereby made a specific part of this resolution.
-SECTION 2: The City Commission of the City of Tamarac approves a 4% rate increase
to Crawford & Company's fee schedule effective December 1, 1994.
SECTION 3: All resolutions or parts of resolutions in conflict herewith are hereby repealed
to the extent of such conflict.
SECTION 4: If any clause, section, other part or application of this Resolution is held by
any court of competent jurisdiction to be unconstitutional or invalid, in part or application,
it shall not affect the validity of the remaining portions or applications of this Resolution.
SECTION 5: This Resolution shall become effective immediately upon its passage and
adoption.
