CITY OF TAMARAC, FLORIDA
RESOLUTION NO. R-92- v2y
A RESOLUTION AWARDING A BID FOR FIVE (5)
AUTOMATIC ADVISORY DEFIBRILLATION SYSTEMS,
BID NO. 92-01; AND PROVIDING AN EFFECTIVE
DATE.
WHEREAS, bids were advertised in the Fort Lauderdale
News/Sun Sentinel, a newspaper of general circulation in
Broward County, Florida, on Tuesday, January 7, 1992 and
Tuesday, January 14, 1992; and
WHEREAS, bids were opened on Friday, January 24, 1992
and
WHEREAS, Physio-Control Corporation , 11811 Willows Road
NE, P.O. Box 97023, Redmond, Washington 98073-9723, is the
lowest fully responsive and responsible bidder.
NOW, THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE CITY OF
TAMARAC, FLORIDA:
SECTION 1: That the bid of Physio-Control Corporation,
11811 Willows Road NE, P.O. Box 97023, Redmond, Washington
98073-9723, in the amount of $32,952.50 for Bid No. 92-01, for
the Tamarac Fire Department for five (5) Automatic
Defibrillators and Accessories Grant Project M-1176, is HEREBY
APPROVED.
SECTION 2: That the funds are to come from Account
001-223-522-647 entitled Fire Department Special Equipment.
SECTION 3: That the appropriate City officials are
hereby authorized to execute any and all account transfers and
contract documents in connection with the awarding of the bid.
SECTION 4: This Resolution shall become effective
immediately upon adoption.

Motion for City Council to award a contract to Physio-Control Corporation in the
amount of $32,952.50 for five (5) Automatic Advisory Defibrillators.

These units will allow trained Fire Department members to treat patients in cardiac
fibrillation in place of or prior to the arrival of Broward County EMS.
STAFF'$_ RECOMMENDATIONS
Staff recommends that the City Council award this contract to Physio-Control
Corporation for the delivery of five (5) Automatic Advisory Defibrillators in the amount
of $32,952.50. Bids came in lower than expected and will allow the purchase of five
(5) units rather than four (Ref: Attached memo to John Kelly).

F NDING DATA
Funding for this project is from a State EMS Grant (M-1176) for $17,400.00. The
City was required to match this for a total of $34,800. The purchase of this
equipment will allow for disposable supplies to be purchased at the same time.

following special conditions (if checked) apply to this bid.
BIDDERS REQUIREMENT FOR CER71FIED CHECK OR BID BOND: The City shall require from all Bidders, a
certified check or bid bond in the amount of five (5) percent of the bid. which shall be forfeited to the City in the event the Bidder
to whom the business is awarded, shall fail or refuse to comply with the specification of the bid. Checks will returned promptly
to the unsuccessful Bidders after award of bid. Failure to submit this bid bond with the bid could cause rejection of the bid.
PERFORMANCE BONDS: The City shall prior to the execution of the contract, require the Bidder to furnish bonds covering
the faithful performance of the contract and the payment of all obligations arising thereunder in such form and amount as the
City may prescribe and with such sureties secured through the Bidder's usual sources as may be agreeable to the parties. If such
. bonds are stipulated, the premiums shall be paid by the Bidder. The Bidder shall deliver bonds to the City no later than the date
of execution of the contract.
