CITY OF TAMARAC, FLORIDA
RESOLUTION NO. R-90-- %v2-
A RESOLUTION AWARDING BIDS FOR GROUP
LIFE AND ACCIDENTAL DEATH AND
DISMEMBERMENT INSURANCE TO NORTH
AMERICAN T.J FE ASSURANCE COMPANY,
BID #89--47; AND PROVIDING AN EFFECTIVE
DATE.
WHEREAS, the City requested and opened sealed bids for
employee group health and life insurance coverage; and
WHEREAS, these bids have been analyzed by the Personnel
Director and Risk Management Consultant.
NOW, THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE CITY
OF TAMARAC, FLORIDA:
SECTION 1: That the City of Tamarac hereby awards Life
and Accidental Death and Dismemberment Insurance to North American
Life Assurance Company at a rate of $.31 per $1,000 of life
insurance, and $.05 per $1.,000 of Accidental Death and Dismember -
ment Insurance.
SECTION 2: That the City of Tamarac, along with its Master
Application, will submit a City check which represents the esti-
mated first months' premium as shown in the bid proposal.
SECTION 3: That the rates of $.31 per $1,000 of life
insurance and $.05 per $1,000 of Accidental Death and Dismemberment
Insurance are guaranteed for a 26 -month period.
SECTION 4: That said rates and insurance shall be in full
force and effect November 1, 1989.
SECTION 5: This Resolution shall become effective immedi-
ately upon adoption.
PASSED, ADOPTED AND APPROVED this day of 91990.

on or before closing hour and date shown above.
