RESOLUTION NO. 3 -fig
A RESOLUTION AWARDING A BID FOR GROUP LIFE -
HOSPITALIZATION - MEDICAL INSURANCE FOR
ALL CITY EMPLOYEES AND DEPENDENTS.
WHEREAS, the City has heretofore advertised for bids
for Group Life - Hospitalization - Medical Insurance for all
City Employees and Dependents, and has received bids, and,
WHEREAS, the Council is desirous of awarding the bid
for such to the lowest and best bidder.
NOW, THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE
CITY OF TAMARAC, FLORIDA:
SECTION 1 : That the bid of Aotoy- L, �s

a copy of which is attached hereto and made a part hereof
by reference, is hereby adopted.
SECTION 2: That such plan being renewable from year
to year this Resolution authorizes the continued payment
in full of such premium, as the same may be budgeted and
billed in subsequent years, unless further action is taken
by the Council to the contrary.
PASSED, ADOPTED AND APPROVED

(3)
All benefits shall become effective December 11 1973 for all full time
employees who have been in the employ of the City prior to September 1,
1973.
(4)
Contract shall provide for automatic coverage (on notification of City
of Tamarac) on the (90-th) ninetieth day of employment for all full time
employees employed subsequent to September 1, 1973.
(5)
Home Life Insurance Company shall agree to service all claims.
(6)
Home Life Insurance Company shall provide:
(1) Individual Identification Cards
(2) Summary Highlights of plan
(3) Printed booklets
