RESOLUTION NO. R-89- Z T
A RESOLUTION AWARDING A BID FOR PERSONNEL HEALTH
EVALUATION FOR THE FIRE DEPARTMENT WELLNESS
PROGRAM, BID NO. 89-23; AND PROVIDING AN
EFFECTIVE DATE.
WHEREAS, bids were advertised in the Fort Lauderdale
News/Sun Sentinel, a newspaper of general circulation in
Broward County on April 22, 1989 and April 29, 1989; and
WHEREAS, bads were opened on May 3, 1989; and
WHEREAS, Willix Health Institute is the lowest fully
responsive and responsible bidder.
NOW, THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE CITY OF
TAMARAC, FLORIDA:
SECTION 1: That the bid of Willix Health Institute, in
the amount of $6,000.00 for Personnel Evaluation for the Fire
Department Wellness Program, with funds to be taken from Fire
Department Account #001-223-522-54B, is HEREBY APPROVED.
SECTION 2: This Resolution shall become effective
immediately upon adoption. //_�
