R-89- 2 � 5 A RESOLUTION APPROVING REVISION OF SECTION 27 OF THE PERSONNEL MANUAL ENTITLED 6 "INSURANCE" FOR THE EMPLOYEES OF THE CITY; AND PROVIDING AN EFFECTIVE DATE, 10 11 12 13 14 15 F[i 22 23 24 25 26 27 28 29 30 31 34 35 WHEREAS, the City Manager has presented to the City Council a revised Section 27 entitled "Insurance" to the Personnel Manual for the Municipal Employees; and WHEREAS, the City Council has reviewed the revised Section 27 of the Personnel Manual and now wishes to adopt said revision. NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF TAMARAC, FLORIDA: SECTION 1: That the revision of Section 27 of the Personnel Manual entitled "Insurance" for the Employees of Tamarac presented by the City Manager to the City Council, is hereby adopted, an official copy of this revision being attached hereto as "Exhibit 1". From November 1, 1988 through September 30, 1989, the following carriers, Cigna (HMO), HIP (HMO) and Florida League of Cities (free choice of physician) will be the only options an employee may choose If they desire to be covered by a group health Insurance plan. If an Individual Is eligible for Medicare, he/she may choose to select Medicare as his/her primary provider in Ileu of one of the Clty's group health Insurance plans. If an employee desires to waive his/her group health Insurance coverage and seek Insurance as a private Individual, the City will contribute the single coverage rate of $41.55 bi-weekly toward the Individual's private Insurance. If an employee desires to waive his/her family coverage, the City will pay $67.15 DI -weekly. In order f0 an employee to be compensated In this fashion, he/she must submit to the Personnel Department an Invoice from the health Insurance carrier they have chosen. Unless there Is an actual expenditure of money by the employee for medical Insurance, there will be no reimbursement by the City. The health Insurance rates from November 1, 1988 through September 30, 1989 are listed below showing the City and the employees bi-weekly contribution. • 1 INSURANCE INSURANCE CARRIER CIGNA (HMO): PAID BY CITY BI-WEEKLY R-17-47spoo SECTION 27 PAGE 2 OF 2 PAID BY EMPLOYEE BI-WEEKLY Single coverage $41.55 $ -OW Family coverage $67.15 $ 42.96 H.I.P. (HMO): Single coverage $41.55 $ 5.26 Family coverage $67.15 $ 69.28 FLORIDA LEAGUE OF CITIES: Single coverage $41.55 $ 22.04 Family coverage $67.15 $118.32 The city has the right to select and change health Insurance companies In Its sole and exclusive discretion. However, alternative cost containment plans may be offered. Such decision shall not be subject to the GRIEVANCE AND ARBITRATION PROCEDURES. The City shall make available optional health and life Insurance coverage for retiring employees. This Section of the Personnel Manual will be subject to annual review. 27.01 GROUP HEALTH INSURANCE : For up-to-date Information on hospitalization and major-medical benefits, an employee should refer to the Insurance schedule booklet or contact the Personnel/Insurance Department. 27.02 GROUP LIFE INSURANCE AND/OR ACCIDENTAL DEATH BENEFITS : The amount of term life and accidental death and dismemberment Insurance, Is the amount stipulated by the Insurance schedule In relation to the employee's annual salary and classification.
