A RESOLUTION URGING THE DESIGNATION OF
4 UNIVERSITY OFFICE PARK, 7800 UNIVERSITY
DRIVE, TAMARAC, FLORIDA AS THE SITE CHOSEN
5 FOR THE HOUSING OF A KIDNEY DIALYSIS CENTER
IN THE CITY OF TAMARAC; AND PROVIDING AN
EFFECTIVE DATE.
8 WHEREAS, the City of Tamarac has received verification that
g a need exists within the City of Tamarac for a Kidney Dialysis
10 Center to be located for general public use: and
u WHEREAS, the City wishes to have said center located in the
12 City of Tamarac; and
13 WHEREAS, it has been established that University Office
14 Park, located at 7800 University Drive, Tamarac, would provide the
15 necessary location and access sufficient for public use.
16 NOW, THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE CITY OF
17 TAMARAC, FLORIDA:
SECTION 1: That the City of Tamarac has no objections to a
Kidney Dialysis Center being located in the City of Tamarac which
20 would meet all the guidelines of the Florida Department of Health
21 and Rehabilitative Services.
22 SECTIQN 2: That a Certificate of Need is to be obtained
23 from the Department of Health and Rehabilitative Services.
24 _ SECTION 3: That the owner of the property known as
25 University Office Park, 7800 University Drive, will comply with all
26 regulatory.requirements and all zoning requirements within the City
27 of Tamarac.
28 E ION 4: This resolution is for the specific purpose of
29 concurring that University Office Park, 7800 University Drive, is
go the location chosen to house the Kidney Dialysis Center in the
1 City.

SECTION 5: This Resolution shall become effective imme-
diately upon its passage.
