CITY OF TAMARAC, FLORIDA
RESOLUTION NO. R-87-_Zl
A RESOLUTION URGING DR. MOHAN GUPTA, DOCTOR
OF INTERNAL MEDICINE TO SEEK APPROVAL FROM
THE FLORIDA DEPARTMENT OF HEALTH AND
REHABILITATIVE SERVICE FOR A CERTIFICATE OF
NEED FOR A DIALYSIS CENTER IN THE CITY OF
TAMARAC, IN AN AREA WHICH WOULD BE PROPERLY
ZONED; AND PROVIDING AN EFFECTIVE DATE.
WHEREAS, the City of Tamarac has received verification
that need exists within the City of Tamarac for a Dialysis
Center to be located for general public use; and
WHEREAS, the City wished to have said center located in
the City of Tamarac; and
WHEREAS, Dr. Mohan Gupta has submitted the necessary
information which indicates that a need for the Dialysis
Center is urgent; and
WHEREAS, the City has been informed by Dr. Mohan Gupta
that it is necessary for the City Council of the City of
Tamarac to show interest and to acknowledge the need for a
Dialysis Center located in our area.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF
THE CITY OF TAMARAC, FLORIDA:
SECTION 1: That the City of Tamarac has no objections
to a Dialysis Center being located in the City of Tamarac
which would meet all the guidelines of the Department of
Health and Rehabilitative Services.
SECTION 2: That a Certificate of Need is to be obtained
by Dr. Gupta from the Department of Health and Rehabilitative
Services.
SECTION 3: That Dr. Gupta has appropriately filed all
necessary documents with the State of Florida Department of
Health and Rehabilitative Services, and will comply with all

regulatory requirements and all zoning requirements within
the City of Tamarac.
SECTION 4: This resolution is for the specific purpose
of concurring with Dr. Gupta's opinion that a Dialysis Center
is needed within the City of Tamarac, and is in support of
same.
SECTION 5:• This resolution shall not in any form or
manner represent that the City of Tamarac is limiting the
rights of other qualified individuals for applying to HRS for
the operation of a Dialysis Center in the City of Tamarac.
SECTION 6: This Resolution shall become effective
immediately upon its passage.
