A RESOLUTION AUTHORIZING EXECUTION OF A
GROUP AGREEMENT TO OFFER AND REMIT HEALTHCARE
OF BROWARD, INC. (HEALTHCARE); AND PROVIDING
AN EFFECTIVE DATE.
WHEREAS, Resolution R-82-81 permitt.ed Healthcare of Broward,
Inc., a recognized health maintenance organization (HMO), to provide
services to employees of the City of Tamarac; and
WHEREAS, Healthcare wishes to continue providing services to
City employees; and
WHEREAS, the City Council of the City of Tamarac desires to
permit its employees to continue to be provided with services by
Healthcare.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF
TAIVIARAC , FLORIDA:
SECTION 1: That the appropriate City Officials are authorized
to execute a group agreement to offer and remit Healthcare of
Broward, Inc., in the form attached hereto -and made apart hereof at
the following monthly rates:
Employee Only $ 58.11
Employee and Dependents 158.00
SECTION 2: This Resolution shall become effective upon adoption.

GROUP AGREEMENT TO OFFER AND REMIT
HEALTHCARE OF BROWARD, INC. (HEALTHCARE)
This Agreement defines the conditions under which HealthCare will be offered as a health plan alternative to
eligible group employees and sets the amount, period and conditions under which its premiums will be remitted for
enrolled members. It is to be understood by all parties that this Agreement limits the financial liability of the group
and the subscriber to the timely payment of all premiums and service fees (as set forth in this Agreement and the
corresponding Group Subscriber Agreement) which premiums and service fees shall be considered as payment in
full for allservices and supplies provided andlor arranged by HealthCare.

ADDRESS
I. Group for itself and on behalf of its employees who meet the eligibility requirements specified in Schedule A of
this Agreement and in the HealthCare Group Subscriber Agreements hereby agrees to offer the HealthCare group
health plan alternative to its employees. A copy of the applicable Group Subscriber Agreements are annexed hereto
as Exhibit 1 and made a part hereof.
II. Group agrees to add the following riders to the Group Subscriber Agreement benefits. (Riders are available only
on a group selected basis; all employees choosing HealthCare must participate in the rider(s) selected by Group):

III. Group agrees to cause to be paid to HealthCare by payroll deduction or otherwise, on or before the first day of
the month for which services are to be provided, the charges specified in Schedule B of this Agreement and Article X
of the Group Subscriber Agreement on behalf of each employee who elects to be covered under the HealthCare
Group Subscriber Agreement.
IV. The effective date of coverage under this Agreement shall be 12:01 a.m. on the first (1) day of April - ,
198 3 , and such coverage shall continue until terminated pursuant to the terms of the Group Subscriber
Agreement or this contract may be terminated by the Group or HealthCare on an anniversary of the effective date
by giving at least ninety (90) days written notice to the other.
V. After the initial enrollment the anniversary date of this Agreement shall be the anniversary date of the Group's
existing health benefits program unless otherwise provided.
VI. HealthCare will conduct an annual reenrollment period, and it shall be of sufficient length and conducted in a
manner so as to meet the requirements for Federally qualified Health Maintenance Organizations. During this time
eligible employees may transfer their membership from the existing health benefits program to HealthCare's Group
Subscriber Agreement or from HealthCare's Group Subscriber Agreement to the existing health benefits program.
The effective date of transfer shall be the anniversary date.

SCHEDULE A
Every employee within the classification set forth below shall be eligible for coverage for himself/herself and family
dependent(s) on or after the Effective Date, if he/she shall have completed the period of continuous employment set
forth in such classification.

Initial premiums shall be payable on or before the first of the month for which services are to be provided and no
coverage shall be in effect until such payment is received by HealthCare. Subsequent premiums shall be payable in a
like manner. Healthcare may change the monthly amount of premiums per employee as of any renewal date of the
HealthCare Group Subscriber Agreement upon giving at least thirty (30) days prior notice to the Group, or
whenever the terms of the HealthCare Group Subscriber Agreement are changed by amendment, by law or by
mutual consent.
