RESOLUTION NO. R-83-_J�V
A RESOLUTION APPROVING AN AMENDMENT TO
THE GROUP MEDICAL/LIFE INSURANCE WITH
FLORIDA MUNICIPAL HEALTH TRUST,FUND -
BID NO. #81-7
BE IT RESOLVED BY THE COUNCIL OF THE CITY OF TAMARAC,
FLORIDA:
SECTION 1: That the Amendment to an Agreement with Florida
Municipal Health Trust Fund, pursuant to Bid #81-7 approved by
Resolution No. 81-54, as amended by Resolution No. R--82-87, is
hereby approved to provided monthly renewal rate of:
Employee $58.11
Dependent 99.71
Medicare Supplement rates shall remain the same
Life and A.D.& D. rates shall remain the same
SECTION 2: These rates shall remain in effect for a
minimum period of three (3) months from April 1, 198-3.
