A RESOLUTION APPROVING AN ANENT TO THE GROUP
MEDICAL/LIFE INSURANCE WITH FLORIDA MUNICIPAL
HEALTH TRUST FUND - BID NO. #81-7
BE IT RESOLVED BY THE COUNCIL OF THE CITY OF TAMARAC, FLORIDA:
SECTION 1: That the Amendment to`an Agreement with Florida Municipal
Health Trust Fund, pursuant to Bid #81-7 approved by Resolution No. 81-54,
is hereby approved to provided renewal rate of:
Employee $ 44.70
Dependent 76.70
Family TTTT .
Medicare Supplement $25.50
Life and A.D. & D. rates shall remain the same
SECTION 2: These rates shall remain in effect for a minimum period
of six (6) months from April 1, 1982.
PASSED, ADOPTED AND APPROVED this 24th day of March, 1982.
